A reference-and-certificate packet for Texas-commissioned notaries and signers whose document provider has requested an acknowledgment. It contains six certificate types for individuals, attorneys-in-fact, corporations, LLCs, partnerships and other representatives, plus a separate notary instruction page.
What you'll need
The complete underlying document, not just this blank certificate.
The required notarial act identified by the signer or document provider. A notary may explain the difference but must not choose the act for you.
A Texas-commissioned notary and the identification needed for your appointment. Leave notary-only fields for the notary.
How to complete it
Download the unchanged packet. Read the reference page, but do not attach that instruction page to the client's document.
Confirm the required act with the receiving agency or document provider before selecting a certificate. Use the acknowledgment certificate matching the signer's stated capacity.
Appear before the notary with the underlying document. A signature may already be on the document, but the signer must appear and acknowledge it.
The notary completes the certificate after performing the act, identifies the attached document, signs and seals it, and securely attaches only the completed certificate page to that document.
Summarize the existence of a Texas trust and the trustee's relevant powers for a recipient. Check the actual trust instrument; a certification does not create a trust or expand the authority it grants.
Document the entity's approval of the specified action and the person authorized to carry it out. Follow the governing documents and required corporate or LLC approval process rather than treating notarization as approval.
A Texas jurat packet for a notary to complete after administering an oath or affirmation and witnessing the signature. It includes single-affiant and multiple-affiant certificates plus separate notary instructions. A blank certificate alone does not notarize an affidavit or replace its underlying statement.
Choose an agent and the financial powers you want that person to have under the Texas statutory durable power of attorney. Review the packet's initialing and effective-date choices; medical decisions belong in a separate medical power of attorney.
